STEPS TO GETTING STARTED

  1. To begin this exciting process please click on the appropriate links below. If therapy will be for an adult, please print out and complete the Adult Form and the Financial Policy. If therapy will be for a minor please print out and complete Child Forms one and two, and the Financial Policy. These forms allow us to ensure that we are providing optimal care and can design a comprehensive program that will best fit each patients needs.
  2. Call or email our office to set up a Visual Therapy Evaluation with Dr. Hohendorf. This comprehensive exam is designed to diagnose the specific nature of the patients visual problem. This exam is not considered to be an eye health examination and we suggest that you visit your usual eye doctor for this type of exam. If you do not currently have a provider, please click here for a map of recommended optometrists in your area.
  3. After the initial evaluation Dr. Hohendorf will provide a diagnosis and set up an appointment for further perceptual testing if needed. This will ensure a more accurate diagnosis and allow us to design a curriculum that will best treat the visual portian of the problem.
  4. Once the perceptual tests have been reviewed a consultation will be set up where the diagnosis and test results will be explained in detail. This will also be a forum to address any specific concerns and/or questions you may have about visual therapy as a treatment.
  5. Finally, we match the patient with a therapist and begin therapy!
